Acute bilateral simultaneous angle closure glaucoma after topiramate administration: a case report

چکیده
INTRODUCTION A case of severe acute bilateral angle closure glaucoma with complete visual loss after oral topiramate therapy. CASE PRESENTATION A 34 year-old woman developed bilateral severe visual loss 2 days after doubling the dosage of topiramate. Her best-corrected visual acuity (BCVA) was counting fingers in both eyes (OU). Intraocular pressures were 49 mm and 51 mm of Hg in right and left eyes respectively, with conjunctival chemosis, corneal edema, shallow anterior chamber and closed angles on gonioscopy. B-scan ultrasound revealed annular peripheral choroidal effusions in both eyes. CONCLUSION Intraocular pressures and anterior chamber depth were normalized after discontinuation of topiramate and initiation of antiglaucoma therapy. Two weeks later, visual acuities improved to 20/25 in the right eye and 20/40 in the left eye. B-scan ultrasound showed resolution of choroidal effusion. Topiramate, an oral sulpha-derivative medication is known to cause ciliochoroidal effusions, which lead to forward rotation of the ciliary body and displacement of the lens-iris diaphragm, with resultant acute angle closure glaucoma and myopic shift.

Acute myopia and angle closure glaucoma from topiramate in a seven-year-old: a case report and review of the literature
BACKGROUND A case is reported of acute bilateral myopia and angle closure glaucoma in a 7-year-old patient from topiramate toxicity. This is the second known reported case of topiramate induced acute angle closure glaucoma and third known reported case of topiramate induced acute myopia in a pediatric patient. CASE PRESENTATION This case presents a 7-year-old who had recently begun topiramate...

CASE REPORT We present an interventional case report of a rare occurrence of acute angle-closure glaucoma in a 35-year-old woman presenting 1 week after start of oral topiramate therapy for depression. Intraocular pressure measured 57 mm Hg OD and 56 mm Hg OS, and bilateral shallow anterior chamber and closed angles were observed. Ultrasound disclosed ciliochoroidal detachment, a closed angle w...

CASE REPORT We describe a 42 year-old patient who developed acute myopia and closed-angle glaucoma one week after beginning treatment with topiramate. Ultrasound biomicroscopy revealed a bilateral angle closure and choroidal effusion. The clinical findings resolved with withdrawal of the topiramate. DISCUSSION Topiramate may cause acute myopia and closure angle glaucoma in some patients due t...
متن کاملBilateral acute myopia and angle-closure glaucoma in a migraine patient receiving topiramate: a case report
We present a case of topiramate-induced angle-closure glaucoma that was treated with cycloplegia. A 40-yearold woman with a history of migraine presented with bilateral acute onset of blurred vision and headache. She had been prescribed 50 mg of oral topiramate bid for migraine prophylaxis 10 days prior to her presentation. On her ocular examination visual acuity was 20/20 with a myopic correct...
متن کاملTopiramate-associated acute, bilateral, angle-closure glaucoma: case report.
This paper describes a topiramate induced acute bilateral angle-closure glaucoma. This rare adverse effect is an idiosyncratic reaction characterized by uveal effusion and lens forward displacement, leading to increased intraocular pressure and vision loss. We describe a 55 year-old white woman with migraine, spasmodic torticollis and essential tremor, who developed bilateral acute angle-closur...
